Dear Victims and Witnesses,
This is an information questionnaire that provides the legal dept with the details that they need to prepare the necessary documents and then legally serve the school principal with a Notification Letter, with all information regarding the incident/incidents attached. Completing this form is the only thing that victims or witnesses are asked to do and the legal dept. does the rest.
The first step is to confirm that the incident meets the standards to be labeled as a bullying incident. Bullying occurs when a student with an advantage in size, strength, stature or standing uses that advantage to harm another student physically or with their words or actions. Bullying may inflict distress or harm on the targeted youth including physical, psychological, social, or educational harm.”
Please, only provide information on incidents that meet the above standards for bullying.
